Effective January 1, 2025, we will be resigning from the Concordia Advantage Network and participating with the National Fee for Service Network with United Concordia and want to let our patients know what that means.
If you are enrolled in the United Concordia National Fee for Service Network nothing has changed.
If you are enrolled in the United Concordia Advantage Network, they will now pay as an out of network provider, and your out-of-pocket expenses may increase slightly depending on your plan. Payment will be due at the time of service, we will submit your claim to United Concordia and they will reimburse you the difference between our fee and their allowance for the services provided.
